When coming for an appointment, please bring:
- contact information from your child's pediatrician or primary care physician
- insurance information and card
- medical summary from your referring physician (if not faxed ahead of time)
Prior to your child's appointment, hip and spine X-rays may be required before Physical Therapy and Occupational Therapy evaluations, as well as gait analysis results.
