Community Health Outreach Worker (CHOW) Program
The Community Health Outreach Worker program was created to assist patients in the management of chronic diseases through in-home support, health education and bridging the gap between the community and the health system.
In partnership with community-based organizations, 15 workers were recruited directly from the targeted communities in New York City. The premise was that familiarity with the local language, values, beliefs, culture, etc., would enhance the worker's effectiveness. Started in April 2003, this program complements the interdisciplinary model of care the ACN ascribes to.
The worker-client relationship has been mutually beneficial and empowering. In a relatively short period, the program has evidenced progress in the following areas: decrease in "no-show" rates; increase in compliance with medication regimen; reduction in unnecessary ER visits and admissions; and improved referrals and communication to primary care site. Finally, workers enjoy a high level of job satisfaction as they witness their clients' health improve.
