Admission - What to Bring
Following is a list of items that are helpful to have when your child is being admitted to the Hospital:
Paperwork and Important Numbers
- Referrals
- Physician names, addresses, phone and fax numbers, e-mail addresses
- Medical history and medications
- Government-issued photo identification (ID) card for parent(s) (Drivers license, passport or other official ID)
- Custody or divorce papers, when needed, to verify who can sign for your child's medical tests and treatments
- Social security numbers of patient and parent(s) or guardian(s)
- Employer ID for patient, parent(s) or guardian(s)
- Emergency contact numbers
- Insurance information (including medical, pharmacy, and dental insurance cards)
For Comfort
- Books, movies, puzzles, water, snacks, and toys
- Comfortable clothes
- Pajamas and slippers
- A small number of comfort items (such as a blanket, or toy, smooth surface and washable items recommended, photographs of family, friends or pets)
- Camera
- Toiletries
- Special diets/formulas
- Cooler for breast milk
Any electronic device brought to the Hospital will need to be approved by our biomedical services.
